Serum Cortisol and Glucose Levels after Lumbar Epidural Steroid Injection. A Comparison with Oral Corticosteroids in Patients with Lumbar Radicular Pain

Abstract
Background: Lumbar epidural steroid injections (LESI) are procedures often utilized in the treatment of radicular pain. The purpose of this study was to compare the changes in the serum cortisol and glucose levels between oral steroid treatment and epidural steroid injection.

Methods: The study was a randomized, prospectivetrial. Thirty patients were randomized to either single LESI (n = 15) with triamcinolone acetonide 40 mg, or a tapering course of oral triamcinolone (n = 15). Fasting glucose and serum cortisol concentrations were measured for all patient before treatment, and at 1 month, and 2 months after treatment.

Results: The blood glucose and serum cortisol concentrations showed no significant changes in the LESI group. In the oral steroid group, cortisol levels were significantly increased at 1 month, and returned to normal at 2 months. Serum glucose levels did not change during the 2 month follow-up peroid.

Conclusions: Single and low-dose LESI (triamcinolone 40 mg) did not result in suppression of cortisol or an increase in serum glucose. However, oral steroid therapy did affect serum cortisol levels.
